  1. Complete form
    Provide information according to the instructions on the form.
  2. Submit form
    Submit your documents according to the instructions on the form.
  3. Allow 7 days to process
    F&G will process the request within 7 business days of receipt of all required information.
  4. Confirm
    A confirmation of this change will be mailed to the owner's address on record upon completion.
