Instructions

Follow the instructions to request a systematic payment from your annuity contract.

1. Complete form
   Provide information according to the instructions on the form.
2. Submit form
   Submit your documents according to the instructions on the form.
3. Allow 5-10 days to process
   F&G will process the request within 5-10 business days of receipt if in good order and confirmation of the payment schedule.
   
   Please note: The processing time includes time to mail payment information to the policy owner.
4. Disburse
   Payment will be mailed to the owner's address on record unless approved as a direct deposit payment.
